How to Use this Form

  • This form is required for all Project Grants.

  • This form can be used as part of a Planning Grant to secure permission for future digitization. It is not required for sample digitization.

  • This form should be signed by the copyright owner of any materials to be digitized with MEAP funds. Project teams must identify the creator and copyright owner for each item included in the grant application and any additional materials embedded within. If your project involves more than one set of archival materials, each owner must complete the form.

  • The signed form(s) can be uploaded as part of your application for MEAP funding.

Copyright Permission Agreement - English

This agreement grants permission to UCLA to publish and steward the digital objects created through an MEAP project. If your project involves more than one set of archival materials, each owner must complete the form.

Translated Copyright Permission Agreement forms

  • Archival Partner Form
    The Archival Partner Form is used to confirm collaboration between the project team and the archival owners. The form should be signed by the current collection holders.

  • Host Commitment Form
    The Host Commitment Form is required for all projects and should be used to confirm institutional agreement to serve as a Host Institution.
